This NDIS Service Agreement Template is a Microsoft Word Document that can be downloaded and modified. There are two versions available 1 for Registered Providers (Provider_Business_Hub_Service Agreement) and 1 for Unregistered Providers (Provider_Business_Hub_Service_Agreement_Unregistered_Providers).
We have also included a Schedule of Supports (Attachment A: Schedule of Supports).
Simply add your business name, Participant details and service details. Then save, print and sign by all parties.
The template can be used over and over again with different Participants. Just be sure to update Participant details each time.
